Blame is a close cousin to shame with a moral or ethical component, and should be reserved for situations where such elements exist. Blame is generally inappropriate for normal business situations and should be avoided at all cost.

Responsibility is related to accountability and transparency. If a business is to be effective, the trifecta of responsibility, accountability, and transparency must be embraced wholeheartedly.

The corollary is mistakes cannot be hidden. They must be freely acknowledged and widely shared so the entire organization can learn and not repeat them.

The team member who has the confidence and courage to say “my mistake,” and here is the lesson learned, becomes the teacher-leader who will advance in an organization.
